You searched “glp1 oral” because you want what Ozempic does without the weekly needle. The four GLP-1 pills on the US market aren’t interchangeable, and picking the wrong one can cost you months.

One pill needs an empty stomach and a 30-minute wait every morning, one has no food rules, and two are for type 2 diabetes. If you’ve spent years hearing “eat less, move more,” these drugs work differently: they quiet the appetite signals that make that advice so hard to follow.

Orforglipron (Foundayo) outside the pharmacy. Orforglipron, the drug in Foundayo, is also sold as a research compound. SwissChems lists orforglipron 6 mg capsules, 90 per bottle, for $198.95, with batch test results posted on its site. Know what you’re buying: it’s sold for laboratory research only and labeled not for human consumption. It isn’t Foundayo, it isn’t FDA-approved, and no prescriber oversees it. For reference, 6 mg was the lowest dose in Lilly’s main weight-loss trial, equal to Foundayo’s 5.5 mg step, and it produced 7.5% weight loss over 72 weeks.

Which oral GLP-1 is right for you

No trial has put the Wegovy pill against Foundayo. Each company ran its own indirect comparison, and each says its own pill wins. Novo’s gives the Wegovy pill about 3 extra points of weight loss. Lilly’s gives Foundayo 1.5 extra points in people with diabetes. Neither settles the question, so match the pill to your routine.

  • Choose the Wegovy pill for the most weight loss from a pill, if you can take it on an empty stomach every morning and wait 30 minutes for coffee.
  • Choose the Wegovy pill if you have heart disease. It’s the only weight-loss pill whose label includes a lower risk of heart attack and stroke. That claim comes from the shot’s SELECT trial, not a pill-specific study.
  • Choose Foundayo if your mornings are chaos: night shifts, an early gym slot, red-eye flights, a coffee you refuse to delay, or a handful of other morning meds.
  • Choose Foundayo if you take levothyroxine. Oral semaglutide raises thyroxine exposure by about 33%, and both want an empty stomach. Foundayo has no fasting rule.
  • Choose the Wegovy pill if you rely on birth control pills. Foundayo’s label calls for backup contraception for 30 days after you start and after every dose increase.
  • Choose the Wegovy pill if you take simvastatin or a strong CYP3A4 inhibitor, like some antifungals and antibiotics. Foundayo doubles simvastatin levels and caps at 9 mg with strong inhibitors.
  • Choose the Ozempic pill if you have type 2 diabetes. Foundayo isn’t approved for diabetes in the US yet.
  • Choose a Zepbound shot if maximum weight loss matters more than the route. Its 20.9% beats every pill.
  • Talk to a specialist first if you’re pregnant, planning a pregnancy, breastfeeding, or have a personal or family history of medullary thyroid cancer or MEN 2.

Wait, Rybelsus is now the Ozempic pill?

Yes, in the US. Novo reformulated Rybelsus into 1.5, 4 and 9 mg tablets that absorb better and renamed them Ozempic tablets. They reached pharmacies on May 4, 2026. If your pharmacy told you Rybelsus is “no longer made,” this is what happened.

The new numbers don’t match the old ones. New 1.5 mg equals old 3 mg, new 4 mg equals old 7 mg, and new 9 mg equals old 14 mg. Two new 4 mg tablets aren’t 8 mg of anything you’ve taken before, so don’t stack them.

Outside the US, the EU and UK approved the Wegovy pill in mid-2026, and the UK approved Foundayo for diabetes before the US did.

Do oral GLP-1s actually work?

Yes, both weight-loss pills beat placebo by a wide margin. The catch is which number you’re reading. Every trial reports two results: one counts everyone who started, including people who quit, and the other counts only people who kept taking it. Headlines pick whichever looks better.

13.6% against 11.2% is a fair comparison, and so is 16.6% against 12.4%. If a page says the Wegovy pill gives 16% and Foundayo gives 11%, it’s comparing two different things.

In the Wegovy pill trial, 60% of people lost at least 10% of their body weight and about 28% lost 20% or more. On Foundayo’s top dose, 55% lost at least 10% and 18% lost 20%.

The month-four checkpoint

In a Novo analysis of the pill trial, about 29% of people had lost 10% by week 16, and that group averaged 21.6% by week 64. Forums often quote the 21.6% as if it applied to everyone. It applies to that early-responding group, but it does give you a checkpoint: if you’ve barely moved by month four, tell your prescriber.

Pill or shot?

The Wegovy pill lands close to the Wegovy shot, Foundayo a few points lower, and no pill touches Zepbound yet. The pill wins on no needles and no fridge. The shot wins on one decision a week instead of seven.

Coming off Zepbound because insurance dropped it? Expect a pill to hold your weight more than drop it further. In Lilly’s switch study, people moved from Zepbound to orforglipron regained about 5.7 lb over 24 weeks, against about 20 lb for those moved to placebo. People coming from the Wegovy shot held almost all of their loss.

  • Commercial insurance: if your plan covers it, savings cards bring either pill down to as little as $25 a month.
  • Medicare: the GLP-1 Bridge covers both pills for $50 a month from July 2026 to December 2027. You qualify with a BMI of 35 or more, 30 or more with heart failure, uncontrolled blood pressure or kidney disease, or 27 or more with a past heart attack or stroke, prediabetes or peripheral artery disease.   • Watch the middlemen: forum users report telehealth membership fees of around $149 a month on top of the drug. Ask what the drug costs and what the membership costs before you sign up.

Semaglutide is a peptide, and your stomach digests peptides. SNAC makes a small protective pocket around the tablet, and the drug crosses right there. Food, coffee, other pills or a big glass of water break that pocket. That’s why the Wegovy pill label puts only 1 to 2% of each tablet in your blood, and why the pill holds 25 mg a day while the weekly shot holds 2.4 mg. Foundayo absorbs like an ordinary pill, per its FDA label.

Wegovy pill, Ozempic pill and Rybelsus

  1. Take it when you wake up, on an empty stomach.
  2. Swallow it whole with up to 4 oz (120 mL) of plain water. Don’t split, crush or chew it.
  3. Wait at least 30 minutes before food, coffee, any other drink or other pills.
  4. If you miss a day, skip it. Never take two.

More water means less drug. In the label’s absorption studies, people absorbed more with 50 mL than with 240 mL. Absorption also rose the longer people waited to eat, tested out to two hours, which is why many users wait 45 to 120 minutes. Treat 30 minutes as the minimum.

Anything you swallow counts. One user’s pill seemed to start working only after they stopped brushing their teeth right after dosing. That’s one person’s story, but toothpaste water, gum and mouthwash all break the plain-water rule.

A habit from people who do this daily: keep the pill and a measured glass of water on your nightstand, take it at 3 or 4 AM, and go back to sleep.

If you take levothyroxine

A common order is the GLP-1 pill first, levothyroxine 30 minutes later, then food. Some prescribers move levothyroxine to bedtime instead. Either way, ask for a TSH check a few weeks after you start, since the label notes higher thyroxine levels.

Foundayo

Take it any time, with or without food, and keep the time consistent. Some users who took it at bedtime felt they were “sleeping through the benefits” and moved it to lunch.

Grapefruit isn’t on the label. Trial participants were told to skip it, which started the rumor, but Lilly doesn’t consider it a meaningful interaction.

Splitting and every-other-day dosing

People do both to save money. The semaglutide label says swallow whole because SNAC works around an intact tablet, so a split one absorbs unpredictably. Every-other-day dosing gives lower drug levels and has never been tested. If cost is the problem, ask your prescriber about staying on a lower dose that still works.

Side effects, including the ones nobody mentions

Stomach effects lead, and they spike at each dose step. In the Wegovy pill trial, 47% had nausea and 31% vomited, but only 7% quit for side effects, against 6% on placebo. On Foundayo, 60 to 69% had some GI effect and up to 10% quit.

Forum reports add a few that rarely make it into trial summaries:

  • Fatigue on Foundayo. It’s the most common complaint among Foundayo users, and hair loss is listed among its common side effects.
  • A “sunburn” feeling on the skin at 25 mg of the Wegovy pill. Some people step back to 9 mg. The label itself suggests the 1.7 mg shot if 25 mg isn’t tolerated.
  • Sulfur burps and a bitter tablet taste on semaglutide pills.
  • A faster pulse. Foundayo raised heart rate by about 4 beats per minute in the head-to-head trial.

Both pills carry a boxed thyroid tumor warning. It’s about medullary thyroid cancer and MEN 2, not ordinary hypothyroidism, so taking levothyroxine doesn’t rule you out.

Foundayo went from filing to approval in 50 days, and the FDA required follow-up studies on liver injury, heart events and thyroid cancer. Pfizer dropped two earlier GLP-1 pills over liver signals. Lilly reports no liver signal so far.

Tell the anesthetist before any surgery or sedation, because both drugs slow your stomach.

Type 2 diabetes and the “GLP-1 inhibitor” myth

Your US pill options for diabetes are the Ozempic pill and Rybelsus. At the top dose they lowered A1c by about 1.1 to 1.4 points in the PIONEER trials and dropped weight by roughly 3 to 4 kg, per the Rybelsus label.

The bigger result is SOUL. In 9,650 people with diabetes and heart or kidney disease, followed for about four years, oral semaglutide cut heart attacks, strokes and cardiovascular deaths by 14%. No other GLP-1 pill has its own outcomes trial.

Orforglipron beat oral semaglutide head to head. In ACHIEVE-3, A1c fell 1.91 points against 1.47 for the 14 mg Rybelsus dose, and weight fell 9.2% against 5.3%. The trade was that twice as many people quit for side effects, about 10% against 5%. US decisions on Foundayo for diabetes and a 25 mg Ozempic tablet are both expected by the end of 2026.

There’s no such thing as an “oral GLP-1 inhibitor”

GLP-1 drugs switch the receptor on. Blocking GLP-1 would raise your blood sugar, so nobody sells that.

What people usually mean is a DPP-4 inhibitor: sitagliptin (Januvia), linagliptin, saxagliptin or alogliptin. These pills slow the breakdown of your own GLP-1, so your levels rise a little. In a meta-analysis of 29 trials, they lowered A1c by about 0.7 points and left weight unchanged. If you were put on “a GLP-1-type pill” and the scale hasn’t moved, read the name on the bottle.

What isn’t an approved GLP-1 pill

Only four pills are FDA-approved. Everything else falls into one of these groups:

  • Research-grade orforglipron. The same molecule as Foundayo, sold for laboratory use, like the SwissChems capsules above. Orforglipron itself absorbs well by mouth, but these products aren’t made to drug standards for people, and their quality rests on the seller’s testing.
  • Sublingual semaglutide drops and dissolving tablets. They lack SNAC, and the FDA says they’ve never been studied in clinical trials.
  • Compounded “oral GLP-1” kits. The FDA warned 30 telehealth companies in March 2026 and about 25 more in June.
  • “GLP-1” supplements, berberine and probiotics. Berberine lowers blood sugar a little. It doesn’t cause the weight loss GLP-1 drugs do.

What happens when you stop

Most of the weight comes back. People who stopped the Wegovy shot regained about two-thirds of their loss within a year. In Lilly’s switch study, people moved to placebo after stopping their injections regained about 20 lb in 24 weeks. Nobody has published stop-and-watch data for a pill yet, so plan for month 13 before month 1.
