There is no best peptide for anti-aging. For skin, sunscreen and a retinoid have far stronger human evidence than any peptide. Oral collagen and cosmetic peptide creams come next, with mixed results. For everything else, no peptide has been shown to slow aging in people. Semaglutide has the best outcome data, and those data cover heart events. The table ranks 13 peptides against that skin baseline by the human evidence behind each.

How to read it. Strong: randomized trial in people, on the stated outcome. Limited: small or mixed trials. Very limited: cell, animal or tiny uncontrolled studies. None: no trial on that outcome.

Do anti-aging peptides work?

Not in the ways most pages claim. None of these peptides has an approved anti-aging use, and no trial shows that any of them slows aging in people. Skin has the most human data, from oral collagen, cosmetic peptides and topical GHK-Cu. Three of the peptides here are approved for other conditions. They are semaglutide, tesamorelin and elamipretide. The rest rests on cell and animal work and small human studies. A 2026 review of nine peptides in gerontology, including tirzepatide, epitalon, GHK-Cu and BPC-157, found the same split. FDA-approved agents had safety data from large trials. The others had preclinical and limited clinical evidence and lacked long-term safety data (40).

What works for skin?

Daily broad-spectrum sunscreen cut skin aging by 24% against discretionary use in a randomized trial of 903 adults over 4.5 years (1). Topical tretinoin improved photoaged skin in a double-blind, vehicle-controlled trial. Fourteen of 15 treated faces improved and none of the vehicle faces did, and skin irritation was the main side effect (2).

GHK-Cu is the peptide on this list with skin data. In aesthetic medicine, 18 of its 20 studies are preclinical and 2 are randomized trials (9). A second 2026 review notes that a 13-person study after laser resurfacing was negative on objective measures, and says GHK-Cu remains unproven as a clinically validated treatment (10).

That makes GHK-Cu a reasonable optional add-on after sunscreen and a retinoid. It doesn’t replace them. Our GHK-Cu copper cream guide compares the creams.

Do collagen peptides work for aging skin?

The evidence is mixed. A 2025 meta-analysis of 23 randomized trials with 1,474 people found better skin hydration, elasticity and wrinkles overall. The trials not funded by pharmaceutical companies found no effect, and neither did the high-quality trials. The authors concluded there is no clinical evidence yet to support collagen supplements for skin aging (3).

A 2026 systematic review of 25 trials read the data more kindly. Collagen beat placebo for hydration in 10 of 15 trials, for elasticity in 10 of 13 and for wrinkles in 9 of 10. Most of the trials had a high risk of bias, and side effects were uncommon (4).

A third meta-analysis, of 19 trials with 1,341 people, found better hydration and brightness and a modest effect on wrinkles that came mostly from oral peptides. Results for elasticity and density were inconsistent (5).

Newer single trials are small. In a 2025 trial, 77 women took 5 g of collagen peptides or placebo daily for 12 weeks. The collagen group had better dermal density, hydration and water loss through the skin than the placebo group (41). In a six-week trial of 80 women using a branded low-molecular-weight collagen peptide, wrinkle volume fell 46% and wrinkle area 44%. Wrinkle depth fell only 9%, and firmness didn’t change significantly (42). Our guide to supplements that help collagen production covers the options.

Do Matrixyl and Argireline work?

Cosmetic signal peptides are the oldest peptide skin products. Palmitoyl pentapeptide (Pal-KTTKS, sold as Matrixyl) has the best-known trial. In it, 93 women aged 35 to 55 used a moisturizer with 3 ppm of the peptide on one side of the face and the plain moisturizer on the other for 12 weeks. Wrinkles and fine lines improved more on the peptide side (6).

A 2023 trial of 21 women compared acetyl hexapeptide-3 cream, palmitoyl pentapeptide-4 cream and placebo around the eyes for eight weeks. The authors called it an initial study and said the pentapeptide did better (7).

Most other cosmetic peptides have thinner data. A survey of products marketed in 2011 and 2018 found palmitoyl tetrapeptide-7, palmitoyl oligopeptide and acetyl hexapeptide-8 were the most used (43). A 2017 review notes that results for many cosmetic peptides are sparse or unpublished (44). Blends muddy it further. A 2025 study of 31 people using a cream with seven peptides plus silybin reported better wrinkles and firmness, but a blend like that can’t credit any one peptide (45).

Argireline (acetyl hexapeptide-8) is sold as a “Botox-like” peptide. A 2025 review says it penetrates the skin poorly, and that whether a cream can deliver it to the nerve-muscle junction remains uncertain (8). Our guides to biomimetic peptides for skin and peptides for skin tightening compare products.

Is semaglutide an anti-aging peptide?

No, but it has the largest outcome trial of any peptide here. Wegovy is approved for weight management and to reduce major cardiovascular events in adults with established cardiovascular disease and obesity or overweight. It also has an accelerated approval for MASH with liver fibrosis (13).

In SELECT, 17,604 adults aged 45 or older with cardiovascular disease and a BMI of 27 or more, but no diabetes, got 2.4 mg of semaglutide weekly or placebo. Over a mean follow-up of 39.8 months, cardiovascular death, nonfatal heart attack or nonfatal stroke occurred in 6.5% on semaglutide and 8.0% on placebo (hazard ratio 0.80) (11). Novo Nordisk funded the trial.

That outcome is cardiovascular disease, which rises with age. SELECT didn’t measure aging. The only randomized data on biological aging come from a post hoc analysis of 84 adults with HIV-associated lipohypertrophy over 32 weeks. Blood epigenetic clocks ran slower on semaglutide, with DunedinPACE 9% slower. The authors call it exploratory and say prospective trials are needed (12). The brain didn’t follow. Two phase 3 trials in 3,808 people with early Alzheimer’s disease found that oral semaglutide did not slow decline, and both were stopped (46).

Weight loss on semaglutide includes lean mass. A systematic review of six studies found lean mass losses from almost 0% to 40% of the total weight lost, with larger losses in larger trials (14). Wegovy carries a boxed warning for thyroid C-cell tumors seen in rodents, and it is contraindicated with a personal or family history of medullary thyroid carcinoma (13). Our peptides for weight loss guide covers the GLP-1 drugs.

Do tesamorelin and sermorelin help with aging?

Both are analogs of growth-hormone-releasing hormone (GHRH). Tesamorelin has a randomized trial in older adults. In it, 152 people aged 55 to 87 (66 with mild cognitive impairment) injected 1 mg of tesamorelin or placebo daily for 20 weeks. Cognition scores improved on tesamorelin, IGF-1 rose 117% and percent body fat fell 7.4%. Fasting insulin rose 35% in the mild-cognitive-impairment group, and 68% of people on tesamorelin reported adverse events against 36% on placebo (15).

FDA approved tesamorelin only for reducing excess abdominal fat in HIV-infected adults with lipodystrophy. The label says long-term cardiovascular safety hasn’t been established, and it warns about neoplasms, raised IGF-1 and glucose intolerance (16). Using it for aging is off-label, and the trial scored cognition without measuring disease or lifespan.

Sermorelin is an older GHRH analog. FDA had approved its brand, Geref, for children with growth hormone deficiency and for a pituitary function test. The maker discontinued it in 2008, and FDA later determined that it wasn’t withdrawn for safety or effectiveness reasons (47). Our GHRH peptides guide covers the class.

Do CJC-1295 and ipamorelin help recovery, sleep or body composition?

No trial says so. CJC-1295 with DAC raised growth hormone 2 to 10 times and IGF-1 1.5 to 3 times in two randomized trials of healthy adults. Those trials measured hormone levels, not sleep, recovery or body composition (35). Ipamorelin’s one efficacy trial, in 117 people after bowel surgery, didn’t beat placebo on its main measure (36). No published or registered trial has tested the two together.

FDA lists safety concerns for both, including increased heart rate with CJC-1295 (19). In UK Biobank data, higher IGF-1 went with a higher risk of four cancers and a lower risk of two (37). Our CJC-1295 and ipamorelin guide has the details.

Is BPC-157 an anti-aging peptide?

No. It’s discussed for tendon and gut healing. The human evidence is three uncontrolled studies covering 30 people, rated very low certainty (24). It isn’t approved. In July 2026, FDA’s advisory committee voted 8 to 6 to recommend it for the compounding list, and that vote is advisory (25). See our BPC-157 vs TB-500 comparison.

Do epitalon and pinealon slow aging?

A 2025 cell study found that epitalon lengthened telomeres in normal cells through telomerase, and in breast cancer cells through a different route (26). Epitalon also raised neuronal-marker gene expression 1.6 to 1.8 times in human gingival stem cells (48). A 2026 review says human evidence is limited to small biomarker studies and an older controlled study in retinitis pigmentosa, with no adequately powered randomized trial (27). A 2024 case report of a 79-year-old man on epitalon, Semax, plasma exchange and stem-cell exosomes recorded a 7.9-year drop in biological age and a telomere length rise from 6.45 to 6.59 kb. With four treatments combined, the report can’t say which one mattered (28). In aged monkeys, a 2025 review reports that epitalon and a related peptide normalized melatonin and cortisol rhythms. The review focuses on work from the Khavinson group, which developed these peptides (49). FDA lists epitalon among withdrawn compounding nominations (19). The advisory committee voted 7 to 4, with one abstention, to recommend it, and that vote is advisory (25).

Pinealon rests on cell work, rat studies and a small Russian human study. In test-tube experiments it interacted with DNA fragments (29), and in cells it reduced oxidative stress (30). A study of 32 patients tested pinealon and a second tripeptide, Vesugen. It reported better biological-age scores, with Vesugen showing the larger effect, along with pro-oxidant activity and lower blood stem-cell markers (31). The design doesn’t isolate pinealon’s benefit. No controlled trial shows it slows aging. Our Pinealon page covers it.

Does thymosin alpha-1 help immune aging?

The thymus shrinks with age, and T-cell output falls with it. Thymosin alpha-1 is a thymus peptide studied as a way to offset that. A 2025 review says studies show it can improve vaccine response in older adults, and it asks for long-term efficacy and safety data in geriatric patients (20).

The trials are small. A 1989 double-blind trial gave 90 men aged 65 to 99 thymosin alpha-1 or placebo twice weekly alongside a flu vaccine (17). A 2007 review of the trials describes enhanced vaccine responses and calls for more work (18).

No trial shows it slows aging. FDA hasn’t approved it, and the agency lists it among withdrawn compounding nominations because the safety information is inadequate to understand the extent of any safety issues (19).

Is elamipretide (SS-31) an anti-aging peptide?

No. Elamipretide targets mitochondria, and FDA approved it for a rare disease. On 19 September 2025 the agency gave Forzinity (elamipretide) accelerated approval for Barth syndrome in patients weighing at least 30 kg, based on knee muscle strength. A confirmatory trial is required, and FDA says serious reactions have been reported (23).

In aging muscle, one randomized trial gave a single infusion to 39 healthy adults aged 60 to 85 with poorly functioning mitochondria. Muscle mitochondrial energy capacity rose right after the infusion (P = 0.045 on the percent change), but the effect was gone by day 7 and fatigue resistance didn’t change (21). A phase 3 trial in 218 people with primary mitochondrial myopathy missed both primary endpoints, six-minute walk distance and fatigue (22).

ReNEW, a phase 3 trial in dry age-related macular degeneration with geographic atrophy, enrolled 313 people and lists primary completion for August 2027 (50).

Do MOTS-c, humanin and FOXO4-DRI work in people?

These are the newest names on the list, and the human evidence is thin.

MOTS-c is a peptide encoded in mitochondrial DNA, and its levels decline with age (51). In mice, it improved physical performance at every age tested, and treatment started late in life raised physical capacity and healthspan. In people, exercise raised the body’s own MOTS-c in muscle and blood (32). Human data on humanin are blood-level studies, such as lower circulating levels in coronary artery disease (33).

A phase 2a trial of injected MOTS-c in 120 adults with prediabetes and overweight or obesity is running at one site in China, with primary completion listed for February 2027 (34). FDA says it found no human exposure data on drug products containing MOTS-c (19). Our MOTS-c guide has more.

FOXO4-DRI is a designed peptide that triggers cell death in senescent cells. In a 2017 mouse study it restored fitness, fur density and kidney function in fast-aging and naturally aged mice (38). ClinicalTrials.gov lists no study of it (39).

Are anti-aging peptides safe?

Most have no human safety record. FDA’s list of compounding substances with safety concerns or missing safety data includes injectable GHK-Cu, CJC-1295, ipamorelin, BPC-157, epitalon, thymosin alpha-1 and MOTS-c. It cites specific risks for some, such as increased heart rate with CJC-1295, and says it lacks safety information on others (19). Quality varies too. A 2018 Belgian screening of ten falsified peptide drugs found purity of 5% to 75% for cysteine-containing peptides, plus arsenic and lead (52). Talk to a clinician before using any of them, especially with a cancer history.

Frequently asked questions

What is the best peptide for anti-aging?

None is proven. For skin, sunscreen and a retinoid have stronger human evidence. Among peptides, oral collagen, cosmetic peptide creams and topical GHK-Cu have small or mixed trials on skin.

Do anti-aging peptides work?

No peptide has been shown to slow aging in people. Evidence ranges from mixed trials of collagen and small trials of GHK-Cu on skin to cell and animal work for epitalon, pinealon, MOTS-c and FOXO4-DRI.

Do collagen peptides work for wrinkles?

Results are mixed. A 2025 meta-analysis of 23 trials found gains overall, but not in the high-quality trials or the trials without pharmaceutical funding. A 2026 meta-analysis of 19 trials found a modest wrinkle effect driven by oral peptides.

What are the best peptides for the face?

Sunscreen and a retinoid come first. Among peptides, topical GHK-Cu and palmitoyl pentapeptide have small randomized trials, and oral collagen has mixed results.

Is semaglutide an anti-aging drug?

Not proven. In the SELECT trial, adults with heart disease and overweight had fewer heart attacks, strokes and cardiovascular deaths on semaglutide (6.5% against 8.0%). One exploratory trial found slower epigenetic aging, and semaglutide did not slow early Alzheimer’s disease in two phase 3 trials.

Do anti-aging peptide injections work?

No injectable peptide has been shown to slow aging in people. Tesamorelin improved cognition scores in one 20-week trial of 152 older adults. For CJC-1295, the trials measured hormone levels, not outcomes.

Are anti-aging peptides safe?

Safety data in people are limited or absent for most. FDA lists safety concerns for several of them, and products sold online vary in purity.

Are growth hormone peptides anti-aging?

CJC-1295 and ipamorelin raise growth hormone, but no trial shows they improve sleep, recovery or body composition, and no published or registered trial has tested them together.

Does epitalon lengthen telomeres?

In a 2025 cell study it did, in normal cells and in breast cancer cells. No randomized trial shows that it slows aging in people.

Are anti-aging peptides FDA-approved?

No peptide is approved for anti-aging. FDA approved semaglutide (Wegovy) for weight management and to reduce cardiovascular events, tesamorelin for abdominal fat in HIV lipodystrophy and elamipretide, under accelerated approval, for Barth syndrome. It lists several others among withdrawn compounding nominations, and the advisory committee votes in July 2026 were advisory.

This page is information, not medical advice. None of the peptides covered here is approved for anti-aging.
